How much CBD does your body absorb?
Less than the label suggests, for every oral format. Researchers estimate that about 6% of swallowed CBD reaches the bloodstream on an empty stomach. That figure is an estimate, not a direct measurement.
A 2020 review in CNS Drugs explains why. Absorption from the gut is incomplete, and the liver breaks down about 70 to 75% of the absorbed CBD before it reaches circulation.
Timing is slow too. A 2020 review in Pharmaceuticals puts the time to peak concentration after oral CBD at 1 to 4 hours. That holds for oils, gummies and capsules alike, because all three are swallowed.
Does food change how much CBD you absorb?
Yes, a lot. In a 2020 study in Epilepsia, healthy adults took 750mg of purified CBD with different meals. Compared with an empty stomach, total exposure rose:
- 3.8-fold with a high-fat meal
- 2.7-fold with a low-fat meal
- 2.4-fold with whole milk
A 2018 study in CNS Drugs found a 4.2-fold rise with a high-fat meal. Food did not change how long CBD took to peak.
These studies used purified CBD at doses far above typical retail servings. The practical lesson still holds: take CBD the same way each day, with or without food, so the amount you absorb stays consistent.
Does holding CBD oil under your tongue work faster?
Probably not by much. Many brands suggest holding oil under the tongue for 30 to 60 seconds. No human study we found shows that this beats swallowing it.
A 2021 trial in the British Journal of Clinical Pharmacology compared a sublingual CBD wafer with a CBD oil solution. Absorption was comparable, and CBD peaked about 4 hours after either one. A 2019 review in Expert Opinion on Drug Delivery argues that most cannabinoids placed in the mouth are in fact swallowed and absorbed in the gut.
Holding the oil under your tongue does no harm. Just do not expect it to act like an inhaled product.
CBD oil: best for adjusting the amount

What about water-soluble CBD?
Water-soluble CBD is emulsified so it mixes with water, and small studies suggest the body absorbs it faster. The evidence comes from small trials run with the makers of these formulas.
In a 2019 study of 16 adults, a self-emulsifying formula peaked at 1 hour instead of 3, with 4.4 times the peak level of the same extract in MCT oil. A 2020 pilot study of 10 adults found a water-soluble powder about 4.5 times more bioavailable than an oil. Both studies were sponsored by or linked to the formula makers.
